How to Get an NPI Number

Applying for a National Provider Identifier (NPI) is free and can be done online through the NPPES (National Plan and Provider Enumeration System). Here's a step-by-step guide.

Before You Apply

You'll need the following information ready:

  • Social Security Number (SSN) or Employer Identification Number (EIN)
  • Your practice location address and phone number
  • Your taxonomy code (healthcare specialty classification)
  • State license number(s)
  • DEA number (if applicable)
  • Contact person information (for organizations)

Step-by-Step Application

Create an Identity & Access (I&A) Account

Visit the NPPES website and create an I&A account. You'll need to verify your identity through a multi-step process.

Log in to NPPES

Use your I&A credentials to log into the NPPES application portal.

Select Entity Type

Choose Type 1 (Individual) or Type 2 (Organization) based on your provider type.

Complete the Application

Fill in your personal/organization details, practice location(s), taxonomy code(s), and license information.

Submit and Receive Your NPI

After submission, you'll typically receive your NPI number immediately or within 1-2 business days. A confirmation letter is mailed within 10 days.

Alternative: Apply by Paper

You can also apply by mail using the CMS-10114 form. Download it from the CMS website, complete it, and mail it to the NPI Enumerator. Paper applications take 10-15 business days to process.

After You Get Your NPI

  • Share your NPI with health plans, clearinghouses, and billing services
  • Update your NPI information whenever your practice details change
  • Enroll in PECOS if you want to participate in Medicare
  • Your NPI is permanent - it does not expire or need renewal
